Data controller and contact
The data controller for these responses is Lamina (an ORAD team initiative). For privacy questions, data-access requests, or to withdraw consent, contact privacy@orad.jasonl.ai.
Information collected
The forms collect professional contact information, practice and workflow characteristics, consent choices, and controlled referral attribution. Do not submit patient, case, or clinical information.
Use and retention
Authorized operators use responses for workflow research, beta qualification when separately requested, and future-offer research when contact permission is given.
